WRITING YOUR CASE STUDY

Gather all of your information according to these criteria

AXIS I:  Clinical Syndromes (symptoms)

• This axis described clinical symptoms that cause significant impairment. Disorders were grouped into different categories such as mood disorders, anxiety disorders or eating disorders.
• List all of your criteria here,, use bulleted form
• List the criteria that is required for the diagnosis
• List all of the symptoms that justifies the specific criteria

 Axis II: Personality and Mental Retardation

• List all applicable information for this section
• This axis described long-term problems in functioning that were not considered discrete Axis I disorders.Personality disorders cause significant problems in how a patient relates to the world and include antisocial personality disorder and histrionic personality disorder. Mental retardation is characterized by intellectual impairment and deficits in other areas such as self-care and interpersonal skills.

Axis III: Medical Conditions

• These included physical and medical conditions that influence or worsen Axis 1 and Axis II disorders. Some examples include HIV/AIDS and brain injuries.
• List all applicable information for this section

Axis IV: Psychosocial and Environmental Problems

• Any social or environmental problems that may impact Axis I or Axis II disorders were accounted for in this axis. These include such things as unemployment, relocation, divorce, or the death of a loved one.
• List all applicable information for this section

Axis V: Global Assessment of Functioning

• This axis allowed the clinician to rate the client’s overall level of functioning. Based on this assessment, clinicians could better understand how the other four axes interacted and the effect on the individual’s life.

Case Study

Start with your History, give as much detail as you can.

Explain what you consider to be relevant episodes and explain different incidents that your person experienced because of the addiction/disorder.  Pick a sub-field of psychology, Psychological, Behavioral, Neurological, Cognitive, and Group/Social, to describe what your person went through, is going through, needs help overcoming.  Explain the thought process involved with the area of psychology that you chose. Justify your choice.

Justify what behavior meets the criteria for your diagnosis.  This information should be in the template above now incorporate it into this section.

Conclusion, recap what brought us here, what treatment methods you will need to help move your person forward.  Look at the information regarding multipath models, BioPsychSocial.

Case study incorporates proper use of DSM‐5 criteria

Includes detailed background, family history, and current issues facing the client/patient

Is creative, realistic, appropriate to topic, and demonstrates the working definition of abnormal

Appropriate/reputable sources cited in APA format

Final copy of case study follows guidelines and  instructions

Case Study Assignment The final submission should be in 2 parts.

1: Fill out the assessment template.

• Disorder name, disorder subcategory (if appropriate), & disorder category
• Bulleted signs and symptoms/diagnostic criteria MOST IMPORTANT
• Prevalence (How common? Demographic info)
• Other interesting findings (if any), famous person with disorder, etc.

2: Two page (minimum) case study see example located under your class resources.

• Background information on the person
• Developmental issues the individual may have faced
• The individual’s current plight/situation
• Identify assigned disorder. Research assigned disorder
• Name the disorder followed by DSM-5 criteria and diagnosis and treatment plan
